You can speed along your first visit by having your forms filled out prior to your visit. Upon making your appointment our friendly staff will guide you to an online registration link or they will mail you the paperwork depending on what is most convenient for you.
Make sure to have everything for your appointment ready i.e. insurance card , id, and any meds you are taking previously and currently.
Please make sure to fill out all proper forms so we can receive your medical records from other offices or hospitals.
We request a 24 hour notice of cancellation for appointments, so that we may accommodate other patients. Also if you don’t give us twenty four hours notice or more we will charge you sixty dollars.
